Sublime and Sublative applications for comprehensive facial rejuvenation

Interview with Dermatologist Dr. Ines Verner*

Dr. Verner: Both systems (from Syneron-Candela) are fractional and have less risks than ablative systems. They are more mildly than the traditionally used ablative and nonablative lasers, that cause more side effects and downtime. eTwoTM uses elōs™ technology (a combination of optical energy with bi-polar radio frequency) to enable safe and effective treatments on all skin types and CO2RETM is a fractional CO2 system that enables you to precisely target and effectively treat the skin’s surface, middle, and deep dermal levels plus perform traditional CO2 resurfacing and laser excision of lesions.

 

How does the eTwoTM system work?

 

Dr. Verner: Radiofrequency (RF) is a novel non-ablative technology originally used to target photodamage. It differs from lasers in that it uses an electric current rather than a light source. eTwoTM with RF creates heat in the skin, breaking up the collagen, which starts to rebuild right away and it combines Sublime – for heating deeper dermal layers – with Sublative. RF causes only minor ablation, thus triggering perfect renewal of the dermis. The water within the skin is a good energy conductor. We discovered, that volumetric heating leads to skin tightening. Another important advantage of RF is, that it does not directly stimulate nerves or muscles and can therefore often be used without the need for general anesthetic.

 

What exactly is the combination of SublimeTM and SublativeTM?

 

Dr. Verner: Well, the SublimeTM application uses the combination of infrared light and bi-polar radio frequency energies to remodel the deep dermal layer of the skin by stimulating new collagen growth. The SublativeTM application uses fractionated bi-polar radio frequency technology, which places the heat energy into the upper dermis where it can produce a significant increase in both collagen and Elastin with minimal dermal disruption.

 

What skin conditions can be treated?

 

Dr. Verner: eTwo’s applications provide for a wide range of treatments. You can treat both fine and deep wrinkles, acne scars, scars and striae and you can improve texture, tone and laxity of the skin. You can also treat all skin types, but on darker skin types you always have to use lower energy levels. Another advantage in contrast to deep peels, etc. is, that you have no down time. But the process of new collagen building is a long process (at least twelve months). You have to tell that to the patients and always make pictures about every three months. So far, almost all of my patients were satisfied.

 

We just did a study with 12 patients (4 male and 88 female), mean age 36. All had moderate to severe acne scarring. We did 5 treatments, once a month, leading to remarkable results. Overall, I find the system safe, tolerable and a effective tool for skin rejuvenation.

 

Do you treat only the face, or can one treat other skin areas?

 

Dr. Verner: First of all we treat the face, because there, the skin heals faster than for example the neck. But we also treat décolleté, hands etc.

 

What can you tell us about “Skinhibitions”?

 

Dr. Verner: Skinhibitions stands for the many patients that are inhibited to be treated for several reasons: high cost, pain, down time and bad results. The Sublative system helps our patients to overcome their “skinhibitions”. But what we really need to do, is educate our patients, that they have to be treated earlier to prevent skin aging.
